• Explain the importance of accident and incident investigation
• Explain the purpose of, and reasons for, investigating accidents and incidents
• Give an overview of accident and incident notification and reporting requirements
• Outline the roles and responsibilities involved in the investigation and reporting process
• Explain how to determine the appropriate level of investigation using an incident severity risk assessment
• Describe the key steps involved in the accident and incident investigation process
• Describe typical information gathering requirements
• Describe the requirement for analysis of the causes of the accident or incident
• Describe Root Cause Analysis (RCA)
• Explain the significance of human factors in contributing to accidents and incidents
• Describe how to identify suitable risk control measures
• Describe the requirement for an action plan and its implementation using SMART principles
