• Explain what is meant by the term ‘near miss’ and why they must be reported
• Give an overview of the Safety Pyramid
• Describe the International Safety Management Code’s requirements in relation to incident reporting
• Explain how to report incidents and what information should be included
• Describe how near miss reports can be analysed to identify causal factors and weaknesses in operational procedures
• Explain why crew members may chose not to report near misses
• Describe the role of the Confidential Hazardous Incident Reporting Programme (CHIRP)
