Once you have completed the course, you will be asked a series of questions to check your knowledge and understanding. These are based on the learning objectives for the course and have a pass mark of 80%.
• Define what is meant by the terms 'incident', 'accident' and 'near miss'
• Explain the purpose of, and reasons for, incident reporting and investigation
• Outline the key roles and responsibilities involved in reporting and investigating an incident
• Explain how to determine the appropriate level of investigation using an incident severity risk assessment
• Identify the key steps involved in a typical incident investigation process
• Describe the requirement for analysis of the causes of the incident
• Describe Root Cause Analysis (RCA)
• Describe how to identify suitable control measures
• Describe the requirement for an incident action plan and its implementation using SMART principles
